Individual Donations

Families and friends may make donations for the treatment of specific individuals receiving services from us. All the money donated (100%) will be applied directly to the treatment of that specific individual. There are no “handling fees” or other hidden charges.

Once the individual donation has been received and processed, it will be applied to the recipient’s account and the recipient or his/her family will be notified that the donation has been made, the donation amount and the donor’s name.

It’s fast and simple and there are several ways to make a donation to an individual recipient. All you need to know is the recipient’s first and last name and have your payment information handy.

MAIL OR FAX DONATIONS (credit card) – Click on the link to the left to get a PDF form that you can complete and fax or mail to us with your payment information We accept AMEX, Discover, MasterCard and VISA.

IN PERSON DONATIONS - If you'd prefer, you can stop by our main office during regular business hours to make a donation. We are open from Monday through Friday from 9:00 a.m. to 5:00 p.m., Monday - Friday. The office is located at:
